Investigation & Safety
Tool 01 — Vehicle Accident Investigation Form The SPECS framework on paper. For each incident: Scene, Process & Policy, Equipment & Load, Crew, System. Causal chain drill. Root cause. System recommendations first, operator coaching second. The form is built around one premise: the individual operator is usually the last link in a much longer causal chain. Tool 02 — Incident Findings Report The formal write-up after an investigation is complete. Timeline, SPECS findings summary, conclusion, recommendations. Designed to be read by someone who wasn’t there. Pairs with Tool 01.
Provider Development
Tool 03 — Provider Development Review, Phase 1 Monthly coaching log for providers in a development program. Attendance and documentation tracked against clear thresholds, but the core is a coaching log with the debrief prompts embedded — so an FTO using this form has the methodology right in front of them. Skills tracked on a See / Do / Teach progression. Companion to Training FTOs.
Tool 04 — Provider Development Review, Phase 2 Same structure as Phase 1, higher bar: zero addenda target, On Scene Times coaching added, final advancement recommendation. The Independence Check section asks the question that matters: what can this provider do without you now?
Leadership
Tool 05 — Performance Conversation Record Documents a performance conversation. Four severity levels from coaching through final warning. Structured around what was observed, what was said, and what changes going forward. Not a disciplinary form — a record of a conversation that already happened. Companion to Hard Conversations.
Tool 07 — Supervisor Self-Evaluation Quarterly self-assessment mapped to five areas: leading vs. answering, debrief quality, hard conversation timing, metrics, and crew independence. Ends with one commitment for the next 90 days. Companion to every chapter in this garden — run it quarterly.
Tool 08 — FTO Selection Guide Behavioral interview guide for selecting FTO candidates. Five questions targeting teaching ability, handling a struggling trainee, productive struggle, running debriefs, and consistency under pressure.
